This job hospital is a 100 bed facility that served the local citizens for 57 years. We are dedicated to providing medical care to the 30,000 residents who live within the 800 square miles of our residing county and neighboring counties. Our vision is to have patients choose our hospital over other available options and to recommend our hospital to family and friends.Supported by a staff of more than 40 physicians, we offer a wide spectrum of services including a full service emergency department that is staffed 24 hours a day and a mobile intensive care ambulance service. The hospital's Intensive Care Unit includes 10 beds (six critical care beds and four step-down beds). Other specialty services include inpatient and outpatient surgery, diagnostic imaging, pathology, obstetrics, physical medicine and rehabilitation, intensive care, and respiratory therapy. Physicians in the outpatient department specialize in gastroenterology, oncology, orthopedics, pulmonology, urology and cardiology. Other outpatient services include sleep studies, nuclear stress testing, pulmonary function testing and a catheterization laboratory. Our hospice provides care for terminally ill patients and their families. It is a hospital-based, physician-directed, nurse-coordinated home care approach to the special needs of the terminally ill patient.

JOB SUMMARY: Plans, organizes and directs the provision of the Case Management Services. Establishes policies, procedures, standards and department objectives. Supervises departmental personnel and manages all functions related to Utilization Review, Discharge Planning, Social Work and Case Management processes.
JOB QUALIFICATIONS:
A. Education:
Bachelor of Science in Nursing, Preferred
B. Personal Job-Related Skills:
1. Work requires knowledge of nursing theory and principles, fundamentals of medicine and medical law, as well as anatomy, physiology, medical terminology, coding and statistics.
2. Skills must include the ability to analyze statistical data, prepare reports, develop policy and procedures and provide overall direction to personnel and medical staff to ensure the appropriate admissions and discharges are achieved.
3. Interpersonal skills are required to act as a liaison among all members of the health care team.
C. Licensure, Registry or Certification:
Registered Nurse with current Texas License
D. Experience:
1. Prior Work Experience: Three (3) years in a hospital setting as a UR Nurse, Case Manager and/or Director.
2. Technical Training: Computer knowledge/experience using decision-making and statistical analysis software.
E. ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Develops long and short-term goals for the Case Management Program.
2. Analyzes data and prepares required periodic and statistical reports.
3. Evaluates and determines present and projected needs for the Case Management Program.
4. Participates in the coordination of internal procedures to ensure compliance with rules and regulations established by regulating agencies, Compliance Plan and CQI processes.
5. Initiates departmental policies, procedures and practices consistent with professional standards, legal/regulatory requirements and institutional goals.
6. Ensures that referrals/discharge plans are initiated in a timely manner through interaction with physician, nurses, social workers, and other health care team members.
7. Utilizes advanced clinical nursing skills to assess patient's condition, diagnosis and individualized interventions.
8. Initiates and follow through with early/appropriate discharge planning in collaboration with the health care team.
9. Facilitates communication links among the different disciplines involved in the treatment plan.
10. Collaborates on a daily basis with physicians and other appropriate healthcare providers regarding the client's progress and changing needs.
11. Negotiates care provided with members of the health care team, client and family to ensure timely and cost effective client outcomes.
12. Directs and coordinates departmental budget preparations and monitor the utilization of approved budgeted resources.
13. Selects, hires, orients, develops, evaluates, disciplines and terminates departmental employees in compliance with hospital policy
